Remnant cholesterol = TC - LDL-C - HDL-C; reflects TG-rich lipoprotein cholesterol. Genetically elevated remnant cholesterol (from APOC3 or APOA5 variants) carries 2–3× cardiovascular risk.
Associated risks when out of range: Remnant particles are highly atherogenic and promote arterial inflammation. Elevated remnant cholesterol is an independent cardiovascular risk factor beyond LDL. Strongly associated with insulin resistance and metabolic syndrome.
