A calculated index of insulin resistance from fasting glucose and insulin. HOMA-IR is measured by blood test as part of the Metabolic panel in the HodieLabs Health Blueprint.
HOMA-IR = (fasting insulin x fasting glucose) / 22.5 (when insulin in mU/L and glucose in mmol/L). The <1.5 target reflects observational literature on optimal insulin sensitivity. It is useful for metabolic coaching but is not a formal society-endorsed diagnostic threshold.
Associated risks when out of range: Gold-standard surrogate for insulin resistance in clinical practice, HOMA-IR >2.5 indicates significant insulin resistance. Predicts progression to type 2 diabetes years before HbA1c becomes abnormal.
The evidence base behind how the HodieLabs Evidence-Based Clinical Library interprets HOMA-IR, from randomised trials, prospective cohorts and genetic (Mendelian randomisation) studies. Full citations in the reference list below.
The HodieLabs Evidence-Based Clinical Library interprets HOMA-IR against published clinical authority standards, never arbitrary "optimal" ranges. Measured in index:
Recommended testing frequency: Annual, as part of a comprehensive panel. Single markers are always interpreted in context.
HOMA-IR is measured in every Health Blueprint at our Melbourne Preventative Health & Longevity Clinic, interpreted by a doctor against the guideline-cited ranges above and tracked over time through your HodieLabs membership.

A calculated index of insulin resistance from fasting glucose and insulin.
The HodieLabs Evidence-Based Clinical Library optimal range is <1.5 index, based on HOMA-IR insulin resistance model (Matthews et al.); <1.5 optimal sensitivity, >2.5 insulin resistant in population studies.
Annual testing is recommended for most adults as part of a comprehensive preventative panel; more often if a result is being actively managed.
Evidence-supported levers include: calculate as (fasting insulin × fasting glucose) / 22.5, lifestyle intervention is first-line: weight loss, exercise, carb reduction, sleep optimisation. Interpretation and an action plan should always involve your doctor.
HOMA-IR is measured in the HodieLabs Health Blueprint at our Melbourne Preventative Health and Longevity Clinic at 85 Spring Street, with doctor-led interpretation against evidence-based reference ranges.
