The biologically active fraction of testosterone not bound to SHBG or albumin. More clinically meaningful than total testosterone when SHBG is abnormal. Can be measured directly or calculated from total testosterone, SHBG, and albumin. Free Testosterone is measured by blood test as part of the Hormonal panel in the HodieLabs Health Blueprint.

The HodieLabs Evidence-Based Clinical Library optimal range is Male: 225–725; Female: 6–50 pmol/L, based on Endocrine Society 2018 testosterone therapy guidelines; calculated free testosterone 225–725 pmol/L (male) reference using Vermeulen equation. Female reference 6–50 pmol/L from RCPA-accredited Australian pathology lab intervals.
Annual testing is recommended for most adults as part of a comprehensive preventative panel; more often if a result is being actively managed.
