FT3 is 3–4× more metabolically active than FT4 at the cellular level. Low T3 syndrome (sick euthyroid) occurs in severe illness, caloric restriction, and high cortisol states.

Evidence-supported levers include: ft3 is the most metabolically active thyroid hormone. Reflects actual tissue thyroid activity, low ft3 with normal ft4 may indicate poor t4-to-t3 conversion (selenium, zinc, or stress-related). Interpretation and an action plan should always involve your doctor.
