CT calcium score: your Agatston number.

A CT calcium score (coronary artery calcium, or CAC) is a quick, non-invasive scan that measures calcified plaque in the coronary arteries, reported as an Agatston score. It is one of the strongest available predictors of future heart attack risk in people without symptoms.

Why the calcium score matters

Calcified plaque is a direct sign of established atherosclerosis. A calcium score reclassifies risk beyond standard calculators. A score of zero is strongly reassuring, while a high score identifies people who benefit most from intensive prevention.

What the clinical evidence shows

• MESA (Detrano et al. NEJM 2008): coronary calcium strongly predicted coronary events across ethnic groups and improved risk prediction over traditional factors.
• Budoff et al. (JACC 2007 and later): demonstrated the prognostic value of CAC across large cohorts.
• Guideline endorsement (ACC/AHA): CAC scoring is endorsed to refine risk and guide statin decisions in intermediate-risk adults.

Clinical interpretation

• A score of zero is strongly reassuring in people without symptoms, though it does not detect soft (non-calcified) plaque.
• Higher scores indicate more established disease and generally warrant more intensive risk-factor management.
• Scores are best interpreted as a percentile for your age, sex and ethnicity, not as an absolute number alone.
