A protein marker used primarily for ovarian cancer screening in women. CA-125 is measured by blood test as part of the CatastrophicScreen panel in the HodieLabs Health Blueprint.

GOVERNANCE: USPSTF recommends against screening asymptomatic average-risk women for ovarian cancer with CA-125 or ultrasound because it does not reduce mortality and causes false positives and unnecessary surgery. CA-125 should be restricted to symptom-driven evaluation or defined hereditary/high-risk pathways (e.g. BRCA1/2 carriers), not offered as a routine optional wellness screen for average-risk users.
Associated risks when out of range: Elevated in ovarian cancer but also endometriosis, PID, liver disease, and peritoneal inflammation. Poor specificity as a standalone screening tool, best used for monitoring known ovarian cancer.

The HodieLabs Evidence-Based Clinical Library optimal range is <35 U/mL, based on ACOG/SGO ovarian cancer screening guidelines; CA-125 <35 U/mL normal cutoff. Elevated in endometriosis, fibroids, and malignancy.
Annual testing is recommended for most adults as part of a comprehensive preventative panel; more often if a result is being actively managed.
Evidence-supported levers include: interpret in clinical context. Ca-125 alone should not trigger cancer investigation in low-risk women, serial trending is more useful than a single measurement. Interpretation and an action plan should always involve your doctor.
